Infiltration of inflammatory cells, rich lipid core and thin fibrous cap with little extracellular matrix are often observed in vulnerable plaque compared with stable plaque. This means that ACS is a kind of chronic inflammation of arteries and changes of vascular cell functions are essential in plaque formation of atherosclerosis. Transforming growth factor-β (TGF-b), a multi-potential cytokine, occurs as a latent complex, consisting of mature TGF-β, latency associated peptide (LAP) and latent TGF-β binding protein (LTBP). Main functions of TGF-β are increase of extracellular matrix, suppression of immune system and growth inhibition of cells. This structural and functional unique TGF-β is thought to be vital in the improvement of plaque stability, for its increase of extracellular matrix and anti-inflammatory effect. However, TGF-β stimulates arterial intimal thickening and thrombus formation with plasminogen activator inhibitor-1 overproduction. It needs to try clinical use of TGF-β with intensive care against atherosclerosis.Latent TGF-β complex has three components as stated above. It is already reported that LAP is crucial in the latency of TGF-β complex, but the role of LTBP is not known in atherosclerosis.
